How To Apply (WIC)

Who is Eligible?

• Pregnant women
• Breastfeeding women
• Postpartum women up to 6 months
• Infants and children up to 5 years old

How do I apply?

1. Call Five Sandoval WIC (505) 771-3587 to schedule an appointment.
2. Meet with our program staff to determine eligibility, in which the following documents are required.

Required Documents​

When you arrive for your appointment, please bring the following:

• Your driver's license or ID or Student ID
• Your child/children's birth certificate or
  • immunization card or
  • baptismal record or
  • birth discharge records
• Your phone bill or gas bill or electric bill
• You proof of income (for all family members) in one of the follwing forms:
  • Medicaid eligibility letter or
  • TANF letter or\
  • Food stamp eligiblity letter (SNAP) or
  • Recent pay stubs or
  • Recent tax return or
  • Letter from current employer
